This differs from another cooler I have in that there isn't one larger ice pack but 10 pen or cigar shaped ice packs that can be used as needed. Using more ice packs will keep temps down longer at the expense of storage room, so this lets you decide how to balance storage vs cooling. With all 10 ice packs loaded up, you can fit 1-2 medical pens. I like the flexibility, though it can get a little cumbersome when removing pens. Just tipping over the container will result in most of the cooler pens sliding out and hopefully the medi pen you wanted, if that doesn't happen, then you may end up dumping the entire contents to quickly get to the medi pen. Not a huge deal, the tradeoff of multiple small coolers is a little hassle getting a specific one out easily. It does its main job well, keep meds at the right temp for a long time. Lastly, I ended up putting the unused cooler pens in a freezer bag, just to keep them together.
